Probably an older Vortex R-4.

They have the 'tailfin' because it was supposed to assist the rotator mechanism, which on these was belt-driven.

The newer Vortex R-x's use a gear driven rotator, much like the 400x's.

CJ, where did you get that the older Vortexes used a belt as part of the rotation mechanism?

See Ian Murr's older finned Vortex.

EDIT:

It has a fin, so it's definitely a Vortex, not a 4004.

Yes, the pole was hit by a drunk driver.
Just because you're offended, doesn't mean you're right.

It's amazing how flimsy the rotator shaft is on the whelen rotating sirens. The same thing happened to a 3016 in Texas a few years back. Plus the center of gravity on these sirens is not centered over the pole itself, it's way in the back.

We shook the hell out of the pole the Howler was on so we could loosen it up enough to pull it up (still had to dig out from around it). I don't think a Whelen could take that.
