Dallas State Prison in PA has a system of a 1003 and two 2904s which directly replaced said 1003s. Indeed, 'twas a sad day when they were all gone. Some of the P-10s went off to small volunteer fire departments, such as Paradise, TX.

I can confirm that it died, I spoke directly with the emergency manager about it some time back, he confirmed that it suffered some kind of major failure and wasn't repairable. The new siren is a 135, apparently it isn't hooked in quite it. The 50 was already disposed of. As for active units in the ...

Entirely possible. Sadly all we can really do is speculate, as we don't have any proper photos of that unit in its original home.

They're all 5/6 1000Ts, mix of A2 and C series. First Wednesday at Noon for three minutes of Alert. They're running on one-way Compulert, activated with FSK.
